Ashwagandha: Does It Actually Boost Testosterone and Lower Cortisol?

Ashwagandha is one of the few supplements in the stress and testosterone aisle with real human data behind it. The cortisol story is solid. The testosterone story is smaller than the label wants you to believe.

Ashwagandha is one of the most hyped supplements on the shelf right now, and it's usually sold on two promises: it lowers your stress hormone cortisol and it jacks up your testosterone. So which one is legit and which one is marketing? The short version: the cortisol and stress evidence is genuinely solid, and the testosterone evidence is real but modest — and mostly shows up in specific groups of guys, not everyone who swallows a capsule.

I get asked about ashwagandha constantly, usually by guys who saw a reel promising it would double their testosterone. That's not what the research shows. But unlike most of the stuff in the testosterone-booster aisle, ashwagandha actually has human trials worth talking about. So let's go through what they say — and where they stop.

The cortisol evidence is the strongest part

This is where ashwagandha earns its keep. In a randomized, double-blind, placebo-controlled study by Chandrasekhar and colleagues (Indian Journal of Psychological Medicine, 2012), 64 chronically stressed adults took 300 mg of a standardized root extract (KSM-66) twice a day for 60 days. Serum cortisol dropped about 27.9% in the ashwagandha group versus a much smaller change in placebo, and their scores on stress and anxiety questionnaires fell significantly.

A separate randomized, double-blind trial by Lopresti and colleagues (Medicine, 2019) used a different standardized extract at 240 mg per day for 60 days in 60 stressed adults and found significant reductions in morning cortisol and anxiety compared to placebo. Two different extracts, two different research groups, same basic direction. When you get that kind of agreement across independent trials, you start to believe the effect is real.

Worth being honest about the caveats: several of the strongest ashwagandha trials were funded by ingredient manufacturers, most were done in India, and many ran only 8 weeks. That doesn't make the results fake — the studies were properly randomized and blinded — but it means we're working with promising short-term data, not decades of independent replication.

The testosterone effect is real but small

Here's where the hype gets ahead of the data. Ashwagandha can nudge testosterone up, but the size of the effect depends heavily on who's taking it.

In resistance-trained men, the best study is Wankhede and colleagues (Journal of the International Society of Sports Nutrition, 2015): 57 untrained young men lifted weights for 8 weeks while taking 300 mg of KSM-66 twice daily or placebo. The ashwagandha group gained meaningfully more strength and muscle, and had a significantly greater rise in testosterone. But we're talking a modest increase — on the order of roughly 15% over baseline — not a testosterone transformation. In infertile men, a pilot study by Ambiye and colleagues (2013) reported around a 17% testosterone increase alongside improved sperm parameters, but that study had no placebo control, so I'd hold it loosely.

And in the general-stress trials I mentioned above? The testosterone changes were inconsistent and often not statistically significant versus placebo. Translation: if you're a stressed lifter or a guy with low fertility markers, ashwagandha might move your testosterone a little. If you're an average healthy dude with normal levels, don't count on it doing much.

If your actual goal is raising testosterone, ashwagandha is a rounding error compared to the fundamentals. I'd point you first to how to raise testosterone naturally — sleep, strength training, bodyfat, and not being chronically wrecked will move the needle far more than any capsule.

The extract and dose actually matter

This is the part most people skip, and it's why so many ashwagandha products underdeliver. The clinical results were built on standardized extracts at real doses. If you buy some random 'ashwagandha' with no standardization info, you have no idea what you're actually getting.

  • KSM-66: A root-only extract standardized to withanolides. This is the one used in most of the strength and testosterone studies, typically at 300–600 mg per day.
  • Sensoril: A root-and-leaf extract with higher withanolide content, usually studied at lower doses (around 125–250 mg) and leaned toward stress and sleep outcomes.
  • Dose range that matches the research: roughly 300–600 mg per day of a standardized extract. More is not obviously better, and most trials didn't go higher.
  • Timing: it's not a stimulant, so take it whenever you'll actually remember. Some people prefer the evening because of the stress and sleep angle.

The safety caveat worth knowing

Ashwagandha is well tolerated for most people, and the common side effects are mild — some drowsiness or GI upset. But there's one thing I won't gloss over: there are documented case reports of ashwagandha-associated liver injury. The NIH's LiverTox database lists it, and there are published case series describing people who developed jaundice and elevated liver enzymes that resolved after they stopped.

This appears to be rare, and in reported cases it generally reversed once the supplement was stopped. But if you've got liver issues, you're on medications, or you're pregnant, this is a real 'talk to your doctor first' situation, not a throwaway disclaimer. And if you ever get unexplained fatigue, dark urine, or yellowing — stop and get checked.

Honestly, a lot of what people chase ashwagandha for — feeling calmer, sleeping better — is downstream of stress and recovery. If that's your real problem, I'd put more energy into sleep and performance and a solid foundation of magnesium before I'd hang my hopes on any adaptogen.

The bottom line

Ashwagandha is one of the few supplements in this category I'll call legit rather than bullshit — but for the right reason. It's a decent stress and cortisol tool with real, replicated human data, and it may give a small testosterone bump in stressed lifters and men with low fertility markers. It is not a testosterone booster to build a plan around. If someone's selling it to you as a natural replacement for what your body already does when you sleep, train, and manage stress, that's the hype talking. Use it for what it actually does, buy a standardized extract, keep the dose sane, and know the liver caveat.
