Steroids, TRT, and testosterone boosters aren't the same. 7 things every man should know — including why the FDA reversed its testosterone warning in 2025.

Steroids, TRT, testosterone boosters — most men think they're the same thing. They're not, and confusing them is exactly how you end up either avoiding something that could help you or wasting money on something that won't. Here are seven things every man should know about steroids, testosterone, and TRT.

1. Steroids, TRT, and testosterone boosters are three different things

Anabolic steroids are what you picture with pro athletes and bodybuilders — the stuff that gets people suspended from sports. These are supraphysiological doses of testosterone or androgens, often five to ten times your normal production, and the goal is to push the body's muscle-building limit. TRT is something else entirely: a medical intervention designed to restore your levels back to a normal range, typically for men who aren't producing enough testosterone on their own. The difference between TRT and anabolic steroids is like the difference between drinking water because you're dehydrated and drowning yourself.

Then there are over-the-counter testosterone boosters. I've reviewed a ton of these on my channel and experimented with plenty myself, and honestly, most are a waste of money — the ingredients usually don't have legitimate research behind them, so you end up trusting a company's marketing story and hoping the science backs it up.

2. A lot of the "testosterone is dangerous" research is dated

The testosterone fear largely traces back to a 2010 study published in the New England Journal of Medicine that basically said testosterone causes cardiovascular problems. Doctors freaked out, the FDA slapped a warning label on the products, and for the next 15 years men asking about testosterone were mostly told no, it's dangerous. But the average participant in that study was 74 years old — frail, mobility-limited men who already had a baseline for cardiovascular issues. The dose wasn't titrated to a physiological level; it was way higher. And the whole thing had only a little over 200 people.

Then in 2014, another study using insurance records showed an increased risk of heart attack in the 90 days after starting TRT. What got left out of the headlines is that it compared the 90 days after starting TRT to the 12 months before starting it — not exactly an apples-to-apples comparison.

3. Low testosterone isn't just low energy — it's a mortality marker

This is the one that reframes the whole conversation. We usually talk about testosterone in terms of symptoms — low energy, low libido, trouble building muscle, brain fog — and those are real. But a meta-analysis pooling data from over 40,000 men found that men with low testosterone had a significantly higher rate of all-cause mortality, and specifically cardiovascular mortality, even after adjusting for age, obesity, and lifestyle.

Now, to be honest about what that does and doesn't mean: it does not prove that TRT extends your life — the randomized controlled trial data doesn't show that yet. But it does suggest the risk isn't only in treating low testosterone. There may be a real risk in leaving it untreated, and that's worth an honest conversation with your doctor.

4. In 2025, the FDA removed the warning labels it added in 2010

This is the part nobody's talking about. In February 2025, the FDA issued classwide labeling changes for all testosterone products and removed the boxed warning tied to cardiovascular risk. The reason was the TRAVERSE trial — the largest testosterone safety study ever conducted: more than 5,000 men over 33 months, randomized, double-blind, and controlled, specifically in a high-risk population of men who already had cardiovascular disease or were at high risk for it. The result: 7% of men in the testosterone group had an adverse event compared to 7.3% in the placebo group — no significant difference. The most conservative regulatory body in medicine looked at that and effectively said it had gotten things wrong.

5. There are real risks — here's what to actually monitor

I'm not going to tell you testosterone is risk-free. That would be dishonest, and anyone selling you that is blowing smoke. The same trial that cleared testosterone on cardiovascular events also showed real secondary signals worth knowing about: testosterone can thicken your blood, which raises clot risk; it can increase the risk of non-fatal heart arrhythmias, particularly AFib, which feels like a fast, fluttering heartbeat; and it can raise blood pressure. The good news is that all three are manageable with the same basic steps you'd take on TRT anyway — regular lab work, regular checkups, and a cheap home blood-pressure cuff if blood pressure is a concern.

6. The labs you should actually get — and what "normal" really means

If you've gone to your doctor, been told your testosterone is "normal," and walked away feeling dismissed, this one's for you. Most primary care physicians only check total testosterone, and that's not enough. Here's what a full picture actually looks like:

  • Total testosterone — the overall amount circulating, and the only number most doctors run.
  • Free testosterone — the fraction you can actually use. You can have a decent total and a terrible free number, and that's often where the real story is.
  • SHBG (sex hormone binding globulin) — a protein that binds testosterone and makes it unavailable. High SHBG means less free testosterone even if your total looks fine.
  • LH and FSH — luteinizing and follicle-stimulating hormone. These tell you whether your testicles aren't producing enough or simply aren't getting the signal to, which points your treatment in different directions.
  • Hematocrit — a baseline before you start, so you know whether your blood is getting too thick and whether you need to donate blood.
  • PSA (prostate-specific antigen) — most men old enough to consider TRT are also old enough to be tracking prostate health, and modern bloodwork follows it well.
  • Estrogen / estradiol — yes, you monitor this on testosterone too. Too low is just as much of a problem as too high.

And about that word "normal": the reference range for testosterone is built on men anywhere from 19 to 99 years old. A 45-year-old's testosterone shouldn't look like an 80-year-old's — but a 45-year-old walking around with an 80-year-old's level is technically normal, just nowhere near optimal.

When I started TRT, my own levels were in the normal range — just on the low side of it. When I brought my symptoms to my doctor, he said, "Why don't we get you on the other side of normal?" That's the standard you want. If your doctor won't run a full panel and won't connect your symptoms to your lab work, it's time to find a new doctor.

7. What to watch for if you go to a low-T clinic

The low-testosterone clinic industry has boomed over the last couple of decades. Some are solid places that give legitimate care and real bloodwork. Plenty of others run a business model that should have you running for the hills. Red flags to watch for:

  • They put you on testosterone without doing a full panel first. That's not medicine, that's sales.
  • Every guy gets the same protocol. Dosing has to be titrated to what you specifically need.
  • There's no monitoring schedule. Bloodwork before, testosterone, then follow-up bloodwork — skip that and problems start.
  • High-pressure upsells. You came in to ask about testosterone and suddenly you need HCG, an aromatase inhibitor, a peptide stack, and a $500-a-month program before you've even had a second set of labs.

A solid provider runs comprehensive labs before they prescribe anything, adjusts your dose based on what your bloodwork shows and how you feel, follows up every three to six months at a minimum, and has an honest conversation with you about the risks, not just the benefits. The goal of TRT when it's prescribed is to get you optimal — not to put you on a subscription you can never get off. If something feels off in a clinic, get a second opinion. This is your health.
